Random Puzzle No.6 [GA OVER, ANSWERS POSTED]

For those of you who don't know the origin of my profile picture, this will be difficult, if not downright impossible. Known side effects include: wanting to jump out of a window, screaming at your computer, intense butthurt, and hiding in a corner saying the word "potato" to yourself over and over again in an attempt to calm down but you can't calm down because you are so frustrated and want to tear the world apart and [Insert Destructive Activity Here] everybody.
To remove side effects, consume 2 tonnes of Butthurt Report Forms, and then fill them out, in that order.
