I only have a hard time dealing with very realistic gore and violence in movies, though I noticed the same feeling while reading Berserk manga. Very disturbing stuff in there.
As for games, I almost always have this fact floating somewhere in my head that what I see is just a game, therefore making it hard for me to see gore and violence as "real".

In movies it can get too realistic for me,
but in games it kind of gets more ridicoulous the more it tries to look realistic.

Fake violence almost never bothers me, real violence is a completely different story though.
