...Well, there's an easy way to check that for anyone... but, sure, since you're asking.
Currently 91 games. I always try to keep it under 100 and recently trimmed it quite a lot since I kept liking the games I tried during the Demo Fest too much and adding them. :X

Anyhow, top 10 (which, honestly, come with a lot of "well actually" footnotes):

1. DOOM - I have precisely zero doubt that I would like that one a lot... Which just makes it feel all that much less justifiable to waste it... So pretty much every time I buy a key for it I just sit on it for a while, look at my bloated backlog and go "Fuck it, I'd rather make someone else happy rather than maybe make myself slightly less unhappy for a little bit." and give it away...
2. Devil May Cry 5 - I'm legitimately not entirely sure if I'd be able to run this one. :X
3. The Legend of Heroes: Trails in the Sky Second Chapter.
4. Bloodstained: Ritual of the Night - Yeah, pretty much same situation like DOOOM except I haven't made any giveaways for it... yet. ^_^'
5. Borderlands 3 - While I'm indifferent about the drama surrounding Gearbox, and would absolutely play the game for the gameplay and occasional jokes that hit (just like Borderlands 2), despite its notoriously cringe-worthy story... yeah, I'm not trusting 2K as a Publisher ever again... :T
6. CODE VEIN - After God Eater 2, I'd absolutely love to play any other "Anime Girl Dress Up Simulator (with bonus game inside!)" by that developer. Now if only they weren't so ludicrously expensive...
7. N1RV Ann-A: Cyberpunk Bartender Action - VA-11 Hall-A's sequel(?). It's, uh, it's kinda in Development Hell.
8. Creeper World 4 - Played all the other Creeper World games and they are very good. I have no doubt this one is too. ;P
9. NieR:Automata - ...I am actively doubtful if I can run this one. The game's overall look already makes me unsure if my old-ass laptop can handle it, but Square Enix being involved just makes me think it's yet another of their porting/optimization disasters. So... yeah... an entry in the Top 10 which I stopped risking entering giveaways for, lest it go unplayed due to computer issues... I still haven't removed it though... Just doesn't feel right.
10. Honestly, most of the rest of the Wishlist may as well be Number 10. It's (currently) Blossom Tales: The Sleeping King but almost everything from there on falls under the "I am very interested in this game... but, eh... does it justify adding it to my Library when there's already a huge backlog waiting for my attention too?" umbrella. Actually, so would the first 9 entries...

I'm really into FPS, survival, and roguelike games so my top 10 currently looks like

1. Valheim - I've been told it's like 3D Terraria and I have over 800 hours on Terraria
2. Ember Knights - a very promising top-down roguelike game with Coop! Can't wait to see it get further along early access
3. The Stanley Parable: Ultra Deluxe - The original is still a masterpiece to me. A short game that was so memorable and enjoyable, I really can't wait to see how the Ultra Deluxe stands up to it
4. Oxygen Not Included - With over 300 hours on Don't Starve Together, any Klei game usually piques my interest
5. World War 3 - This FPS will eventually be Free-to-Play thankfully, I'm pretty excited about what this game could be with a healthy chunk of players
6. Cyberpunk 2077 - Not an RPG person (sorry). But Witcher 3's story telling was so incredible, I'm interested in what CDPR can do with a future cyberpunk setting
7. Hades - The quintessential roguelite of recent memory. Supposedly a masterpiece and clearly has the reviews to show for it so I'm quite interested in testing out that theory
8. Scrap Mechanic - As a huge survival game fan, since they added a "Survival" mode, the potential for this game has really skyrocketed, I hope they continue to develop this for a long time. Really wanna get my hands on this one.
9. Ready or Not - I have hundreds of hours on R6 Siege, this hardcore tactical style FPS seems like a really great blend of coop fun but hardcore tactics
10. Inscryption - One of the most unique, card games I've seen. I watched a playthrough and stopped because I felt like I was spoiling some of the really really interesting parts of the game that's hidden throughout that feels best enjoyed by playing it yourself.

My wishlist is a mix of things I really want, as well as things I find interesting and would like to try. Selfie : Sisters of the Amniotic Lens and Droplitz are very much ones I find interesting. Especially Droplitz, because I love puzzle games. Also, some of them are freebies on my list as a bit of a reminder to play them, and some just became free after a while.

My wishlist is probably going to shift a bit, after I play some of these on Game Pass or Itch.io. I already played Hypnospace Outlaw on GP, but it's no longer on there so to the top of the WL it goes!

In the past, I chose games for my my wishlist with the thinking that I would instantly play them if I was gifted them or there was a deep sale, which helped to keep the number down. I used the 'Follow' function for games I was less sure about or wanted to wait and see before I decided. I've changed how I use it a bit and wishlist more freely now. There are still only 81 games on it though, so I guess I'm still quite picky!

Like most people, I have a backlog, so I look for something extra rather than just games that look good - those that that are a little different to ones sitting in my backlog, games I have played demos of and know I will like, or have very, very good reviews. Similarly, my top 10 are quite different to each other. Even with these, I still have some concerns - will I get fed up of reading in Disco Elysium? I have lots of rogue-lite games that I enjoy but never get very far with them so is Hades gonna be the same? etc.
