I am not sure if that fits as answer, but in Vampire the masquarade: Bloodlines beauty/charisma stat affects your flirting and blood drinking possibilities. Also on the other hand if you play as nosferatu monster, you aren't allowed walk out of sewers to talk with normal people... you actually can, but will arrive police and will want kill you for being too ugly.

Also Arcanum, if you're too ugly people won't talk to you, and if you're pretty you get a discount and more conversations, but it's by the same authors, Troika.

south park : fractured but whole does it but its based on the color of your skin (difficulty becomes harder the darker the skin)

Not 100% what you mean, but in Kingdom Come: Deliverance some dialogue changes depending the clothes you wear when you talk to some NPC.

Lots of RPGs have a charisma stat that affects some checks/unlocks some dialogue options. For example the Fallout series, Baldur's Gate, Arcanum: Of Steamworks and Magick Obscura, Neverwinter Nights. Additionally, Arcanum has a specific Beauty stat. Fallout and Arcanum also have traits/backgrounds related to physical appearance: Fallout has a "Sex Appeal" trait and Arcanum has "Super Model" and "Beat With an Ugly Stick" character backgrounds, which provide various bonuses and penalties.

D&D uses "Charisma" explicitly only as an "overall personal appeal/charm" stat and not a "physical beauty" stat (notwithstanding that great beauty normally also means high CHA, of course). In particular I can't recall any instance in BG where high CHA is treated as your character being beautiful (as opposed to just somehow being charming enough to convince people, leaving it implicit how). I mention this because I've specifically played the BG series... a lot... and don't actually recall any instance where this happens, although it's admittedly been a few years and I might just have forgotten. Of course you're free to roleplay that it's your appearance contributing most or all to CHA, but the games can't assume it.

Planescape: Torment is an even better example of a game with the exact opposite: high CHA makes an enormous difference in what kind of options unlock, but since the protagonist is, well, more or less a zombie (with no way to change appearance other than clothes) this all boils down to charm/personality. ...well, except for the bits where you actually interact with other undead, that can get, uh, interesting.

Yes, I didn't mean that the two are the same, simply that charisma could be related to beauty as well as other things. You are right that charisma-based reactions tend to be written so that it's not specifically a reaction to physical beauty, that's also why I mentioned that Arcanum has charisma and beauty separately. And indeed, Planescape Torment is a good example where charisma is based on personality as well.

Yeah - I remember that you could get your car back (and receive free upgrade to it) by having sex with npc if you had right perk + was playing as a female in fallout 2. You could also repeat that activity couple of times in a row and guy would die by exhaustion xD
Also you needed to to have that "beauty" related perk to become a porn star.

Well, Mass Effect 2. Your actions influence your look, so you can either look human or half cyborg. It's a bit the other way around, but as you shape the world, you shape your looks.

Rimworld has a Beauty stat for the colonists wich affects the colonist's opinion of each other.
They can also have traits ranging from beautiful to staggeringly ugly and some body implants can also have an impact.
Then there's also disfigurement like losing the nose or an ear wich heavily impacts the opinion from other colonists. Such injuries often lead to break-ups between couples and fights between the colonists.

Could you explain what youre really after?
I mean southpark changes on Skin color difficulty.
than there are Mobas,RPGs and many other games where Character size/skin can change Attack range or your hitbox.
Or Fable (its pretty long ago but fancier clothes or character beauty points i dunno anymore ,characters would surround you with hearts(you can marry literally anybody there and build your own stock market +Change how the world will change from your actions.
Beyond two souls,Heavy Rain & Detroit become Humans ,Witcher 3,Mass Effect 2 ,Fallout 2, Dragon Age: Origins,Elden ring and many more will change story on your actions..
if you could specify what exactly youre looking for would be easier to say(specially because there are games where i dont wanna spoil things and some reactions would only occur if you do specific changes to your outfit/char)
I just assumed that you dont wanna see any dating games ,where the main focus was charming people with beauty .

I'm not after anything in particular, I just wanted to know games where beauty can actually cause interesting changes, especially in relationships with other characters and story, since sometimes we can waste some time choosing the look of our characters, but most of times it has no impact in the game. Though you're right, I'm not interested in any dating games at all.

The mechanic also exists in Fable as well as in the old classic Black & White 2 (not on Steam). Your character (or your hand and your pet in B&W2) changes visually depending on whether you're acting good or evil.

Gta San Andreas has some minimal changes if you're fat or musculous, like being mocked by pedestrians or having more stamina.

Crusader Kings 2

There's a "handsome" trait that gives a huge bonus to relations with opposite sex characters (or same-sex gay characters), and an "ugly" trait that gives a malus. If you stack that trait with the seduction traits the game becomes super easy, especially as a female character (since most of the important AI characters will be male). Your vassals and neighbors will love you, everybody will want to join your plots, the best commanders from everywhere will want to join your army with no conditions, etc etc.

All I can think of are Pokemon beauty contests in the 3rd or 4th gen haha
Or on a related note, photography/art games where the composition of your shots is rated. But it doesn't have to do with your character's appearance... Unless you play as a photograph o.O
