well first you need to get voted by the people so volvo picks your item to be released to the store
even after you get accepted, it takes a few weeks to months before your item get's put it on store (valve will contact you this, this includes contract << how you get paid, % of valve's cut)

I don't really know the specifics.

Sometimes you get contacted by tournament organizers for your item to bundled (another way to get paid).

I don't like it.

I like it.

However, I hate Dota 2.
