Yes, a few times.
Once because the game went on sale the day after I got it, since I hadn't played it yet I got a refund and bought the Deluxe version.
But most of the time I get a refund because the game just wasn't what I was expecting or wasn't fun at all.

Yes 2 times.
First was for Tomb Raider. Purchased the game, but 3 days later Steam had sale for Tomb Raider GOTY Edition for same price. Asked for refund and bought GOTY edition.
Second was Bad Rats: the Rats Revenge. Bought Inexistence after refund thinking it was better game at the time because of the positive reviews. Learned my lesson - at least let the game have >1000 reviews before purchasing. And also Bad Rats: the Rats Revenge is the better game than any other games as per SG community.

Just recently refunded PLAYERUNKNOWN'S BATTLEGROUNDS. Got sucked in due to the hype and streamers but the game was just utter crap. I didn't even notice any "performance issues" that some other people complained about.
edit: thanks for the blacklist whoever it was :D

I did once. I purchased a game as a gift for someone only to find out after the fact that they owned it on another platform. I ended up returning it and getting them something else. It all worked out.

There was really nice discount for Mount & Blade Warband in my country so wanted to buy a few copies for this site, but you can't buy many at once on steam... so I made a mistake with one and added it to my library instead of inventory, that was my the first and only one refund.

I have refunded twice. Both times when I bought games in a sale and then they were given out in the humble monthly less than 24 hours later. And fuck publishers/developers who think that bullshit is clever.

I think only 2 refunds.
One was because I couldn't get it running (Vanishing of Ethan Carter) on my old machine. I rebought it after I got my new computer and it's a good game.
Other one (Braid) I just didn't like at all. I'd just finished Limbo and loved it, and Braid seemed similar. All those great reviews and I couldn't stand it.
Most of the time I don't play them fast enough to have a chance at a refund.

All the time, honestly. I'm actually really picky when it comes to games. They're not even bad games, but if they can't capture my interest within the refund window, I'm not taking any chances on being stuck with a boring game.

No, but I did try asking for a refund on SG because I haven't won any games
