Sorry, but this quiz is not good. There is absolutely no enjoyment in knowing the correct answers but not being able to guess the specific format you want. I finally got Q1 after a dozen guesses but I haven't been able to guess the format for Q2 yet.
Quizzes should be about knowledge, not formatting. Please consider adding multiple correct answers to account for different formats or, better yet, list the exact format you want.
