I think you might be thinking of darkseed. At least it sounds like it.

Indeed, especially with HR Giger's involvement in Darkseed.
The basic shape he used for the Xenomorph from Alien shows up in plenty of his other works.

I cant remember the name of the game, and I dont know enough detail for google it, so I hope there is someone who can help me with this. So the details that I remember:
The game had side-scroller perspective, the protagonist was male, and the story has something with aliens (similar to the alien movies, but NOT in a scifi environment). I dont know if the alien was the same xenomorph that featured in the movies, but the game was started as the protagonist woke up in a big house, and noticed that he has some kind of illness, and there was also a time limit thing, which if runs out, then a cutscene was shown, where an alien brakes through from the protagonists lung, and game over. Even though the game has this alien thing in it, as far as I can remember, the game was set in the early 1900's, in a small town, where was different places to go (the one is that I remember clear, was a barber shop)
I hope someone can help me, couse I looking for this game for days :)
