Out of the Box has been bundled at least two times, not 0 as in the OP.
Got it from a Fanatical bundle 2-3 months back and its currently bundled again.

Hi all, who didn`t knew it by now - all Mystery bundles are the same bundle with re-freshed names! (to generate new sales from release NEWS)
games just added and removed depending on stock availability.

It's been in 2 Mystery Bundles and one Build Your Own Lockdown Bundle 4.
These "special" bundles should not count as "bundles" because of their high price (the games featured are usually not changed to partial CV either), but in the case of Build Your Own Lockdown Bundle 4 I see that the CV has adjusted, so it is considered a "bundle" as an exception.

Good point, I missed that one, although at the time of making this chart (game first reported on 20 Feb) the Guardian Bundle wasn't released (25 Feb) :D

If I could select games that I don't want to get, because I already got them, maybe those lootbox bundles would have any worth. But well, people are naive, thinking they will get the main prize easily. I am not.
