CV = Steam store price in US$, unless it has been in a bundle before (only 15% of current price) or completely free (no CV at all). Two easy ways are checking the asterisks behind the game's name while creating the GA (*=bundled; **=free) or visiting sites like isthereanydeal.com.

Humble Monthlys are usually not considered as bundles, as the amount of discount isn't big enough (I'm not totally sure which it was, 95% or so, others will hopefully correct me here ^^)

If you are first time subscriber and you have the 10% off coupon or by getting a 1 year subscription. However, the bundle is being sold for 12$ in any other case.
So in your opinion, let's take away from anyone buying it at 12$ 'cause there is a way for some people to get it cheaper. Yeah, it makes no sense.

You not being able to access doesn't mean that the majority can't. The bundle is being sold for 12$ and only under certain circumstances you can get it for cheaper.
It's the same as having a stackable coupon on Steam while there is a game already heavely discounted. You being able to get it for cheaper, has nothing to do with the discount itself.
And even if you get it for cheaper, for 10.8$ lets say, the MSRP of the bundle would still need to be 216$, which it never hits. It only hit more than 200$ twice since it was introduced in late 2015.

I'm not totally sure which it was,
It's "approximately" 95% off USD price in any region- meaning that if you want to cover your bases before making an unbundled giveaway, you should to check the game's SteamDB page for currency conversions against USD, and then see if you're at all close to 95% off.
It's my understanding that such calculations are typically run off the best possible recorded sale price for any bundle, regardless of what the [BTA/non-coupon-price/etc] gets to. It's also worth noting that such calculations are done per tier, within a tiered bundle [which is why Humble Tier 3s rarely get added to the bundle list].
Monthlies- like many bundles- are USD-specific in pricing, meaning you don't have to bother with complex currency conversion calculations with them. Monthlies seem to steadily hover in the mid-80% to just-over-90% discount range, meaning they usually don't get close enough to 95% off to be at risk; so far, we've only had a single monthly get bundled.
