Dog Tag Format

I am ordering a Dog Tag.

Can you people help just me what information will look good on it.

The Dog Tag consists of 2 Plates, Each Plate have 5 Lines that can be embedded.

Why not standard things? like Name, Adress, Blood Type etc.

It is kinda useful if something will happen to you.

EDIT: Oh right... and birth date.

Don't try putting there anything "fancy" like quotes, hobby or stuff like that. It will be lame... I can assure you.

For DOB what should be the Format.
Ex that i like: Year-Date-Month
and what should i use . or - or / to seperate.?

There isn't one valid format of date. It kinda depends on what country are you from.

For example in my country we are using DD-MM-YYYY or DD.MM.YYYY but it is not standard one. It would be probably best if you check what type of dogtags (+what kind of informations) are used in your army.

Second plate: List what you like maybe? (Gaming, paintball, soccer, pie eating...)

btw put the one tag inside one of your shoes in case you get dismembered.

I guess it depends where you live. In my country the format is Date-Month-Year.If your day of birth is bigger than 12 the format wont matter at all. The separator is mostly personal preference or you could write the month with letters. I would use dots, looks nicer.
example:
Solid
Stamos
25.10.1992 or October 10 1992
B positive (lol thats a nice moto)
22 Jump Street,Minas Tirith, Middlearth

last name
first name [space] middle initial
SSN [space] two letter branch code
blood type [ex: o pos]
religion

At least that's whats on my AF ones.

If you want to put a religion but don't have one 'NP' (stands for No Preference) is used a lot.
