Steam key gift card template?

I remember seeing a template for a card you can give to your friend with a cd-key on it on the forum. I tried searching the forum a bit, but i can't find anything about it. I want to give my friend a few games from amazon, but physically. Anybody know where i can find it?

It's A4 - 80g template for home purpose. Just have to glue front with back on one side (makes 160g paper and looks nice)
