Survey for Borderlands 2 [Giveaways]

Just wondering what some good (free) survey making/hosting sites are; have a statistics project and need a huge sample size for a mock pilot study.

There will be a giveaway involved; I was thinking Borderlands GOTY or Endless Space.

edit: Survey is completed, so now I just need to know how to split up the giveaway codes. I don't want to link the survey until I establish the codes and set up the Borderlands 2 giveaways. :P

edit edit: Three giveaways for Borderlands 2; two are private and found within the survey (they are not hard to find). One is public to support Grauntgar's SteamGifts Thanksgiving 2014 Giveaway Event Topic and draw attention for my survey.

SurveyMonkey is free for questionnaire with less than 10 questions, as far as I remember. Or you can just use Google Docs, it also has survey option.

So, I have some Borderlands 2 to giveaway; should I split the giveaway code up between pages or have the five characters at the end of the survey?
