These are my LEAST favorite mods... I don't want to play a game to fap, I want to play a game to get a story... kind of hard to get a story when staring at your partner the entire time...

I'm going to be honest, I never even gotten a full on boner from the mods. They're not really realistic enough to do the job for me. Regardless, as much as I enjoy having them to make the game feel more realistic, my favorite are probably weapon mods. I liked the zombie one from fallout 3 though.

Yeah, I just don't care for nude mods. I'd rather add content and weapons than nude textures... But I've got a reason for it, I just won't get in to it here.

Project Nevada is a must have but download the New Vegas Script Extender (NVSE) first.http://nvse.silverlock.org/ I personally love Nevada Skies too which adds weather effects to the game.

Mojave Music Radio Extended should be a must. (You'll probably need something like Radio New Vegas Extender in order to add all them though).
Because let's be honest, as great as Big Iron is, hearing it like 23 times an hour is a bit much.

Electro-City - Mainly adds a ton of light-posts around the highways and roads in New Vegas - great mod. There is also an option to disable light-posts on highways if you so desire
Nevada Skies - Great weather mod that adds 60 new weather types, ranging from sand storms to rad storms to thunder storms...lots of storms =). Great mod.
Weapon Retexture Project (WRP) - a WIP mod that retextures most weapons in Fallout New Vegas with a HD texture; most of FNV's guns had a lame low-res texture, and this mod corrects that =)
Interior Lighting Overhaul - Overhauls the lighting of all interior cells of (vanilla and all DLCs) to a more realistic and "fitting" value, to increase "immersion and creepiness, especially in vaults, caves and ruins"
Fellout - removes the orange tint from vanilla FNV
Populated Casinos - Many new NPCs are added to the casinos of FNV, giving them a more populated and "alive" feel.
More Perks - Absolutely love this mod. Adds like 100+ cool new perks to the game
Moburma's FNV Restored Cut-Content Mods - which includes the New Vegas strip without loading doors, a bigger more open Freeside, many new NPCs, and other restored cut content and/or unfinished content. Stuff they had to change do to console not being able to handle it.
NMC's Texture Pack - I prefer the small pack with faded textures to match the setting so you don't have to use the 4gb launcher for the full HD textures.

+1 using same mods expect Fellout. The Game looks very good with nevada skies and NMC texture pack. Also try out the Imaginator mod you can tweak some visuals in game ( bloom, color, saturation etc...)

Project Nevada was the mod that i require. Too many nice balancing features and immersive features. I also tend to use WMX, EVE and FO3 restoration mod including a rock-it launcher fix cuz man that gun was fun in FO3. Theres also a mod (i forget the name) that added an underground sewer thats connected map wide which was awesome.
