I also think it marks all messages up to the time when you click the button, even if you have not seen them yet. This means if you leave the messages window open for 10 minutes and then mark them as read, it will mark the messages you received in the past 10 minutes but have not appeared on the page yet because you didn't refresh it.
Edit: I'm not positive, but I think that is how it works.

That would make sense actually. I assume that if that's the case the boolean in the database would simply be put to "true" (for "read" or whatever) for all "false" ones. That would include all new ones, since they are in the database, just not rendered yet.

I'll test it. I have my messages window open with your new message still there. Someone reply to this and after I see the reply here, I will mark my messages as read without refreshing the messages page to see if it marks the new one that I have not see yet as well.

Yup, it marks all messages as read from the time you click on the button, whether or not you have seen them.
I just refreshed this page, saw your message, went to my messages tab that I left open with only the last message showing and not yours, clicked "mark as read" and it marked the new message that was not shown on the page as well.

Yeah, I noticed this a long time ago. I always refresh the page right before I mark them as read to make sure no more showed up while I had the page open and then after I mark them I check the top comment and make sure it was one that I already saw. It's really annoying that you can't just mark one at a time.
Another problem is that if you start a thread and get a ton of replies overnight, you end up with multiple pages of replies and marking them as read will mark all the pages. You want to get through that big group of messages so that you can mark them as read so you know where you are, but as you are replying to people more messages keep coming in. It can be too hard to keep up and you just have to ignore the entire read/unread system like it isn't there and just try and remember who you replied to.
