I'd love to go more towards Cybrog division, and mutation divisons. Prehapes add Genetic engineering and limb system? Like you could lose them entirly, and you'd be force to add cyborg arm. I'd love it to go to the North aswell, I like snow ;D (Dead Snow anybody?)

Zombies. Because everything is zombies these days and you need to have them to bring in the masses. Forget story, forget gameplay, forget engine....just zombies. Have Tamriel go Walking Dead on us and call it a day. Have the protagonist be known as The Governor so he has free reign to kill humans, zombies and children (saw someone above wanted the ability to kill them.)

What I would really like.
More music. Don't get me wrong the soundtrack is great, so are the stations. I would just like, first of, more diversity and a longer song list per radio in general.
A harder and working combat system. It was quite idiotic what the fights ended up being. More so the melee fights. You basically tried to get the enemy into a melee animation and then kited him. Also the old "go on a rock and make the pathing system crazy" trick. You were more focused on metagaming and trying to cheat the system with FPS tricks than a RPG like skill based system. If you were good enough jumper you could kill quite high level enemies no matter your level. Also the enemies closed the distance stupidly fast, like the animation speed was working 2x or 3x of what it should be. I get it, it was done for balance purposes as your range attacks would of been too powerful against melee but the way it was done - well it didn't work at all. So in one sentence the game catered towards a good FPS player, rather than a good RPG player. If you had good skilzors you could mindlessly blast away, took away the strategy part of Fallout games. It made the combat a lot more fast for sizzle but it didn't dizzle my nozzle you wizzle?
Most importantly, Engine. For gods sake. The number one concern when Fallout 3 was announced was the Oblivion engine. Everyone knew the faults. Stupidly bad animations, clunky combat, faces, idiotic physics (props jump up when you enter a room or manipulate nearby items) and the general graphics that were kind of aged by then. Well you could always hope that they would improve Oblivion to Fallout 3 - They didn't. All the faults of the engine were still clearly visible. So please, please make a good engine.
More crazyness. New Vegas already got the gist as it was made by some of the original developers of Fallout 1 and 2 but add even more. It is a huge part what made the first two Fallout's fun.
Bring back the global map. I would like that they would combine the old-school global map and the new idea of a single 3D map. Basically make a global map and then regions you can travel to. Each region of course has its own map. This way you can focus on making each area interesting and diverse as they don't have to logically follow the style of the previous sector. So you can still have big sectors/maps for certain areas with wasteland and nothingness but the regions of the map that aren't made to 3D map allow maptravel as in the old days with all the cool encounters. So we don't have to be in a desert area, you can still travel to a temple or a big town without it looking weird of how the terrain switches. This would allow to bring back those awesome 3 way battles you had when encountering animals, civilians and slavers. Oh and you so 4 way encounters unless you took sides.
Harder game in general. The game is too vanilla shmanilla. I remember how I was blasted to pieces from level 1-3. It was brutal man. Even the first dungeon/tutorial area was semi-hard to complete.

What i would like to see is more radio songs, after some time you get tired of the same music over and over again
More trowable items, like the baseballs you found so often, there is a perk that makes you trow further, however, trowable items are hard to find
Being able to get limbs severed, like arms and legs, growing them again with radiation, or a new kind of chem, using a crutch in case of not having the chem
Being able to choose with what hand you shoot, lefty power you know?
