Help Me Find an Old Puzzle

This ith quiz had an image of 4 curved arrows pointing to the left on the left side and a picture of Death on the right side. The answer was obviously Left 4 Dead. The quiz was released some time in September 2012 or perhaps August.

Thanks chour, but unfortunately that's not the one. The quiz I'm looking for only had one picture in it - the rest were text-based questions.

I know it was August or September last year, because that's when I released my rebus puzzle, and it was inspired by that single question from a couple of weeks before.

Was the prize L4D as well or just the answer to one of the questions?

E: If it's a mashup, White Panda has had panda suits in at least one of their videos that I've seen, although that's probably not it :P

Thanks for the idea, searched for my posts from August 1 to Sept 30, no luck. For some reason, google doesnt actually show all the possible results. I tried looking for Award's puzzles earlier today (site:steamgifts.com/forum "Created by Award") and google missed quite a few of his threads.
