I'm guessing the games have to also be GOOD as well, so that severely limits the selection. Because I've played quite a few very violent games that were also very terrible.

The only game that hadn't been mentioned yet I can think of is They Bleed Pixels. Blood it literally the game's scoring system- you get "pints" of blood instead of "points". XD

Scribblenauts Unlimited. You can create god, feed him with a poisoned rotten meat, set him on fire with flamethrower and after all use his corpse as fountain for kids! Oh, and then you can drop bulldozer on kids. Burning explosive bulldozer!

New Tomb Raider is pritty violent game :)

the Kane and Lynch series is pretty graphic, not good, but violent sure.

Also L.A. Noir has some pretty nasty murder scenes. Nothing you really get to see but examining those panties gave me shivers of disgust... not to be mistaken with shivers of pleasure...

I know of a game who apparently makes the player violent, some sort of subliminal messaging. The player simply goes insane. There's been cases where the player has brutally murdered his/her partner or a family member after playing it. The game has been banned since but it can still be found in the depths of the internet. I won't name it here though.
