So I created a giveaway (https://www.steamgifts.com/giveaway/cGnEN/one-piece-burning-blood), and sent the winner the code - The winner viewed the code - you can see by the little eye on page, then they marked it not received. I checked the activation and Steam says it has been activated on an account, but not the user's who won.
What should I do?

contact the key provider (humble bundle) for a new key
they will send you a new key and provoke the old one (if you lucky)
then you can sort things out with the winner

Sounds like he won the game for a friend, which is against the rules here. Do what the above posters said and then report him for not activating the win.

what do you mean with "checked the activation and Steam says it has been activated on an account", you tried to activate it and it said "duplicated key"?

then sushisu's suggestion could work, if you got the key from a bundle site that can revoke keys.

if not, send a request received feedback ticket with proof you sent the key and where you bought it (screenshots). not sure if this works but it's worth a try.

I sometimes see a winner with dozens of prior wins, and only one unactivated from a mass giveaway, 50+/100+ keys. If that's their only strike, I tend to let it slide, because I know those can go wrong in a number of ways. Am I wrong to do this? Should I always request a new winner in that case?

a lot of the time when there is an old non-activation, they have already received their reprimand ... unfortunately the only way to know is to ask for a re-roll.

Have you checked the user now? It was a GA that ended about 10 days prior to your GA that made question this.

Best you can do is try to contact the winner to find out what happened, can't assume it was them. As stated above contact HB letting them know the issue you had with the key.

Don't make that mistake. You'd be surprised how many shady individuals there are on SG.
If you're cool giving some people games they're just going to sell/trade, then by all means, disregard this message.

But also, keep in mind ... every person you report for rule-breaking on this site is one less headache for the next guy, and you're helping the entire community by reporting them.

There are thousands of rule-breakers here, you should always check your winners. For example, your winner has:

  1. A recent VAC ban.
  2. One not activated win (from this same month)

So probably he's trying to break the rules again. And judging from his profile info, he's not exactly a child.

Having to much trust can backfire because besides good people on SG there are also some very bad and useless ones that will not play by the rules and/or don't care about the rules. I know this out of experience making several hundreds of public giveaways that were from level 0 up to and including level 3. I had winners with multiple non activated wins, winners that marked received and re-gifted, winners with multiple double wins, exotic ratio's, VAC and Game bans, even one with a trade ban.

Now I've transitioned my public giveaways to level 2 SGTools giveaways and it's much nicer to check my winners and to give my gifts to SG users who do follow the major official SG rules and who aren't cheaters and the like.

This is why you should use humble bundle link. It's clear as crystal if the link has been used or not otherwise. Considering the giveaway you created, you had an option to do that.

Well, that was what i thought too. But in one case this also failed for me.
Similar circumstance, winner never said anything and simply marked as not received, never really got an response, but the gift link was used. So i created a ticket to delete GA. Support wanted to also contact the winner, but I haven't heard anything since.
