I had two guys who apparently never resold the won keys or added them anywhere, they just stored them in a big file for future use. One went back over a year with them. And they were never caught by giveaway creators for non-activation. I honestly felt incredibly, unexplainable strange as I tried to explain someone living almost the other side of the planet (Singapore) that yes, you have to activate the damn key on your own account in a week, not just put it on a shelf to decide on its fate.
Not that it applies to this case, just thought I share that sometimes you get the strangest stories of why a game was never activated on an account yet marked as received.

I had similarish experiences when people with 10++ nonactivated games were caught and fixed it in like an hour - likely they had the keys around, and I just couldn't wrap my head around that logic. It's like some benign simplicity like Ralph in The Simpsons, who doesn't want to do bad things, just don't really grasp that he should do the right either.
Edit: oooh, I think I now see why you wrote that. So guy had it on won page, not activated it, got caught, and couldn't activate it...that explains indeed.

I had one key marked as received and not activated....because horrible memory.
Won GA, checked on mobile, knew user, so marked as received with intention of activating after coming to pc....
I was reminded about that some time later by suspension lol :D activated proper key on spot.
So there are some different stories out there :)

Well, they do click "received" there no "activated" option anywhere...
With people's general lack of FAQ/Rules reading and the high change of users not knowing language enough to translate it, there's quite a big chance to catch someone who genuinely will not know that SG's "received" actually means "received and activated".
Not that it excuses them, "Ignorantia legis non excusat", but at least explains how those things can happen.

What you describe is a generic problem and in my experience SEA people are especially prone to commit this error the first few times; heck, some mark the key received immediately, since technically they received it, they just activate it later. (Another part is they really are eager to show appreciation and gratitude, something people a lot closer to my region seem to lack… sometimes entirely.) This is why it was suggested to add a Reveal key button as a step before marking it as Activated instead of Received. Would help a ton.

they can't distinguish whether it was removed from account or not, as far as they were aware it was never activated. for that reason, yes when something gets revoked for whatever reason you should change feedback to not received and explain in a comment on that giveaway why the feedback was changed.
as far as it happening again you would never get suspended for the same exact infraction twice, there is some kind of note on why the suspensions happen to prevent accidentally suspending someone twice for the exact same thing.

In fact, you could have contacted support even after getting suspended
BTW, those keys where invalid, you wouldn't even be able to activate them.
Revoking a game already activated in your steam library would be harder to do. Not to mention you would receive a message stating the game was removed, which could have been used has proof.
