Hello, I have DS4 with PC via bluetooth.
Devices will lost themselves if you'll make pairing/linking before adding device via bluetooth.
Select "Add new device via bluetooth" in Window's bluetooth menu. Select it again at top of device list (best MS's logic). Choose correct category of device. Start searching base device on DS4 by holding PS-key (DS4 should blinking). Choose DS4 in Windows when they'll find themselves (could take up to 10 minutes if you have lot of mess in local network by neighbours). Done. Windows should make device linking/pairing after it that will work continuos time.
I hope I remember procedure correctly.

Does your PC connected to bluetooth device correctly?
I'm using desktop with bluetooth dongle, so Windows automatically installed bluetooth dongle first of all.
P.S.: I spent about 10 minutes to let my PC find DS4. Where're toooo much wifi-nets aroun here, so they make lot of problems for connection even stable.

Usually when a bluetooth device asks for a PIN/password and you haven't got one, you can put 0000.
I use DS4 with bluetooth, on Windows I'm using ds4windows and it works great. It has been years since I set it up so I don't remember if I had to do anything special.

You can start from updating your bluetooth drivers. If it doesn't solve the problem, you can try to search for your ps4 controller manually from control panel>bluetooth (or something like that) and try to add device.
And importatnt: are you using built-in bluetooth or the one that you need to plug it into usb port?

Hmmm, that's weird. Maybe there's something wrong with this USB thing. You can try:

Don't buy USB Bluetooth dongles, I had tried using multiple of them on my PC to connect my PS4 controller and they never worked properly. They either had horrendous input lag or stops responding from 3 feet away.
Solution: If I were you I'll connect the Controller Wired via USB or Wireless using DUALSHOCK 4 USB Wireless Adapter. I bought mine on ebay works great.

Try this. Get it from Samarcanda on steamcommunity, it works to me.
"after you connected the controller to steam through the cable, unplug it, push the PS and SHARE button together for 3 sec until the light start to blink, go to bluetooth settings and add the controller as you would add any other bluetooth device.
After that steam recognizes it also when wireless."

Where did you get the controller from?
I remember i had the same issue with 1 of my PS4 controllers then realized its a fake and the bluetooth on it is non-functional. The other controller i bought after always connected via bluetooth while the previous one never connected on any of my devices

It seems bluetooth on Windows 10 has some issues. I use a wireless Xbox One Cotroller and charge the battery via USB. Whenever I unplug the USB I always have to "Remove Device" from bluetooth then add it back again cause it won't sync.
