A new account starts with 3 GA slots. When your GA marked received by winner you get your slots back. For every 3 successful GAs you get one more slot.
Ahh thx my math was of and I do have one not received due to bad key from dodgy site bundle( I was not going to buy the DLC for the game again for that one)
It couldn't happen to any of us. Did you ask winner to delete GA. If winner agrees and writes that they are ok with it, you can ask support to delete GA. That way you won't have any red not received marks on your user page :)

If I recall correctly you start with 1, then jump up to 5 and after that it gets successively more, I'm at 10 now...

I have done 175 GAs and have a limit of 59 so I think you also get some when you lvl up

Nope. You don't get more slots with levels.
Base Slots + (Received GAs ÷ 3) - Not received GAs = Your GA limit
3 - (175 ÷ 3) - 1 = 60 (Rounded down)
You don't get your slots back if it's not received or deleted. Since you have 59 slots you probably deleted one of your GAs :)
