Does it count the time I already spent on games, or does it sum up the amount of time needed to finish every game as if I never played a single one?

Well most games don't even have achievements so they wouldn't even be used for calculations anyway. Idling wouldn't matter as well (as far as calculation goes). You'd take the time needed to beat the game from HLTB and see if the user playtime is equal or greater than that. If it is you just skip that game entirely and go to the next one. If not you just add HLTB(game) - userPlaytime(game) to the total sum.
As you say, it would be impossible to know if a user idled or actually played the game, but hey, it's something.
