It depends on the brand, how recent they are, and the cost. More expensive ones comes with a lot of adapters, the cheaper ones may not. I had a cheap $30 Sapphire, and a $40 XFX one that both came with a DVI to VGA adapter. Most may not enter this due to the reason they don't have the base game of course. Maybe make the time longer, or just create a public giveaway. :P

as for your question, not all video cards have DVI to VGA converter.

I was wondering why did you ask if all video cards come with it... is there something missing in your video card that you think it should have in the box or you're planning to buy a new video card and wanted the adapter? you shouldn't be using VGA...HDMI or DVI provides the best quality. there's also a display port connection but only few monitors have that kind of connection.

AMD Radeon HD 6800 series came with one.

I never got one. A damn shame, I could use my old monitor with this PC as a second monitor for reading wiki pages when playing games.

I got one with my nVIDIA GeForce 9800 GT. I don't use it because VGA's colors looked washed out and DVI's colors are more vibrant

In answering question number 2, not always.
Many cheaper cards do not include adapters.
And most open boxes & refurbs (like what Tiger Direct loves to sell) rarely if ever come with any accessories.
