Another possiblity: it was a Steam gift and the creator couldn't reach you for whatever reason (email invalid or didn't arrive/ended in spam folder, blocked on Steam...)
It seems unlikely, but could happen I guess.

Oh, since you showed us who is GA creator I take your side. All you can do is mark as not received and report GA creator :)
